By submitting this authorization form, you agree to hold information obtained from the web site confidential and agree that any purchase orders placed via the web site constitute legal purchase orders authorized by your company and carry the same terms and conditions associated with any traditional hard copy purchase order. Please complete the form below and email it to the address provided. We will process your request within 24 hours. Your email address will be used as you user-id and you will receive an email once your account has been approved. You can change your password at any time by using the password options in the “my account” section of the web site.
